Parents as Teachers

Provides screenings to reassure parents when their child is developing on target and identify problems early to assist with appropriate interventions- health, hearing, vision, lead, immunizations and dental are also monitored, and connections to community resources.

Offer office-based individual and family therapy. Also offers in-home or office-based Behavioral Health Intervention Services (BHIS.) South Oaks Residental Treatment

Serves 20 males 12-18 years of age. Each youth receives an individualized assessment and service plan from a Licensed Practitioner of the Healing Arts to help youth develop higher levels of personal accountability, congestive self-control, and personal coping skills.
Stop Outpatient Youth Counseling

Counseling services for young men 11-17 years of age with a variety of needs. Youth may enter the program where planning may indicate the need for long-term structured, group care; or youth may utilize program as a step-down from more structured programming.
Shelter Services

Agency: Four Oaks Family & Children’s Services

Provides emergency and temporary care and supervision to runaway, homeless, delinquent and transitional adolescents. Referred youth may be victims of abuse, neglect, exploitation, violence, emotional abuse or behaviorally disorders. Length of stay is intended to be no longer than 30 days.
Smith Center Residental Treatment

A female/male facility serving 12 adolescents, ages 12-18 years of age. Each youth receives an individualized assessment and service plan from a Licensed Practitioner of the Healing Arts to help youth develop higher levels of personal accountability, congestive self-control, and personal coping skills.
Sexual Abuse Treatment and Re-Orientaion Program (STOP)

A residential program designed for adolescent males ages 12-18 years of age who have sexual behavior problems. Components of the treatment service include family, individual, and group therapy.

Youth Shelter

SUCCESS Program

PHASE programming incorporates state-of-the-art, evidence-based interventions for juvenile offenders. Participating youth are allowed to stay connected to their community, schools, and families, and graduate from PHASE with reduced risk to reoffend.
