    04/14/2020 - COVID-19 Changes:
    Telehealth is now available to all patients. Patients can call and schedule the appointment and will receive the easy to follow instructions for the appointment. This is a safe and secure method to take care of your health during the COVID-19 pandemic. We advise patients to call their home PHC clinic to schedule.

    4/10/2020 - COVID-19 Changes:
    Mental health therapists are available and have openings for Telehealth, and some in person, appointments. Accepts all insurances as well as individuals with no insurance. Also offer a therapist that speaks Spanish and one that speaks Bosnian. MAT services are still available via Telehealth (contact the Marshalltown clinic to schedule). We are still taking MAT clients in person at the Des Moines locations with same day options for Telehealth for mental health therapy services. The Marshalltown phone number is 641-753-4021. The Ames clinic offers these service as well. Their number is 515-232-0628. 

    SSVF is a Veterans Affairs funded program which is delivered through Primary Health Care. This program is focused on helping homeless and unstably-housed Veterans and their families living in Warren, Polk, Dallas, Jasper, Story, and Marshall Counties. Services provided include:

    CASE MANAGEMENT services that will assist participants in obtaining VA and other public benefits, including: health care referrals, daily living referrals, personal financial planning, fiduciary and payee referrals, legal services, housing counseling services, employment and training referrals.

    TEMPORARY FINANCIAL ASSISTANCE (TFA) which may include rent assistance, moving expenses, security and utility deposits, transportation, and child care. All financial services based on eligibility and available funding.

    Intake: Call to set up an appointment and to complete a screening, if no phone is available, client can walk in to set up an intake appointment

    Eligibility: Veteran (and family) experiencing homelessness or in an unstable housing condition with income less than 50% of the median county income, must have at least one day of active service and discharge status with anything other than dishonorable

    Documents Required: Documents must be provided prior to funding dispersment, but are not required at time of intake: Proof of income, SS Card, DD-214 (discharge papers), homeless verification (if staying in shelter)
